BECOME A SERVICE CENTER
To apply for service center status:
- Applicant MUST be a registered and active distributor of Daxen, Inc., preferably at the Star Agent or above status
- Please read and complete Service Center Application Form
- Read the Operation Manual and sign on the last page (signature required)
- An initial Product Order is required for a minimum of US $2000.
Please download the 3 documents, print out and read carefully. After you have completed the Application Form and Product Order Form, as well as signed on the last page of Operation Manual, please send by mail or fax them back to us.
All forms are available from AUToTrack system.
Daxen, Inc. shall review your application and notify you of approval status. Please note that in certain cases, you may not be approved.
